There's a lot of threads asking about Foxdie, so I figured we could put together a definitive answer about it. This will hopefully clear up any questions people have about the FoxDie virus
What is Foxdie?
Foxdie is a virus that is programmed to attack and kill specific people. Just about everyone involved in the Shadow Moses incident was infected with FoxDie. It causes death by simulating a heart attack.
How does FoxDie work?
FoxDie is a virus that goes into macrophages (which are a part of the immune system). After going into the macrophages, Fox uses enzymes (specialized proteins), to attach to the DNA sequence that it's "programmed" to recognize. Once the virus attaches to the specific DNA sequence, the macrophage starts to produce TNF (tumor necrosis factor) epsilon. TNF-epsilon is a cytokine -- a chemical messenger -- that is carried along with the blood, to the heart. When TNF-epsilon gets to the heart, it binds to the TNF receptors, which cause certain pathways within the myocardial (heart) cell leading to its death. This programmed cell death is called apoptosis, as Naomi points out, and is analogous to cells "committing suicide."
It's interesting to note that FOXDIE causes a heart attack, but not like "normal" heart attacks. "Normal" heart attacks (myocardial infarctions) usually happen when a (coronary) artery is blocked, thus depriving the heart cells of oxygen and sugar, causing them to die. However, in both cases, heart cells die.
How does this tie into Solid Snake and MGS1?
Snake was injected with the disease by Naomi before being sent into Shadow Moses. She wanted revenge for Snake's killing of Gray Fox in MG2. However, the Pentagon wanted to use the virus and Snake to take care of the mission
Snake was sent into the mission to expose the Darpa Chief, Armstech President, Foxhound members and genome soldiers to the virus so that the Pentagon would be able to get Metal Gear Rex unharmed. Basically, he was used to spread the virus that would kill everyone so Metal Gear wouldn't be harmed and the terrorists could be killed
But Naomi changed Snake's FoxDie code at the last second. She set it to a wildcard value so no one would not know when it would activate.
What role does FoxDie play in MGS4?
In MGS4 however, she reveals that he has only 6 months to live. Whether or not this is because of FoxDie or some other reason is not known.

Isn't there a vaccine for foxdie?? I just re-played MGS:TTS and at the end Ocelot says "Yes Thanks To The Vaccine" and it sounds like he's talking about foxdie and why he didn't die.
